Student-Athlete Medical Forms
The information located on this page will be required by all student-athletes in order to participate in intercollegiate athletics at Texas Lutheran University. If the information listed below has not been submitted prior to the first day of practice, participation in intercollegiate athletics will not be allowed. Please view the general instructions document for more information.
- Physical (freshman and transfers only)
- Sickle cell trait test (freshman and transfers only)
- Insurance that covers athletic injuries (front and back copy of insurance card required)
- Required information and forms located in SWOL

Sickle cell trait test
A sickle cell trait test can be ordered by your doctor at the time of your physical. For additional information about sickle cell trait testing, please view the general instructions document.
Insurance
Student-athletes must have an insurance policy which provides coverage for injuries sustained during intercollegiate athletics participation and has limits of $90,000 or better per incident. This limit is set by the NCAA. For additional information about insurance, please view the general instructions document.
