Student-Athlete Medical Forms

 

Please review and complete the checklist below based on your classification. All information is required prior to participation in intercollegiate athletics at Texas Lutheran University. This information is also located on the homepage once you log into SWOL in the general instructions document.

Freshman and Transfers

All information will be submitted via SWOL. Please follow the instructions listed in SWOL Instructions. School ID to join SWOL is “TLU”. New accounts will not be approved until after all TLU spring sports conclude post season play which is typically after June 1st.

  • Physical
    • TLU form highly recommended for freshman
    • Transfers may use physical from their previous university
    • Upload on Forms tab in SWOL
  • Sickle cell solubility test, sickle cell trait test, or newborn sickle cell screening*
    • Request from doctor at time of physical
    • Transfers may use sickle cell test from their previous university
    • Upload on forms tab in SWOL
  • Insurance that covers athletic injuries (upload insurance card in SWOL)
  • Required information and forms located in SWOL
    • ID once logged into SWOL is your TLU ID# (301
    • Required information indicated by red text or red text box

Returners

*Sickle Cell Testing
Sickle cell trait testing is completed at birth. These test results can be requested from the department of health in the state which you were born. Your doctor can request these results but it may take a few weeks to receive. This option is usually free. A new test takes a few days to have results returned and is usually a low cost option.

Insurance
Student-athletes must have a health insurance policy which provides coverage for injuries sustained during intercollegiate athletics participation and has limits of $90,000 or better per incident. This limit is set by the NCAA. If you do not have health insurance, you will need to obtain a comprehensive or short term health insurance policy prior to participating in intercollegiate athletics (see below if you need a policy). Supplemental accident and hospital policies to not meet the requirements set by TLU and the NCAA. They may supplement a comprehensive or short term policy however. Policies must be active for both the traditional and non-traditional seasons (the entire school year for most sports).

Need An Insurance Policy?
Many short term insurance policies include exclusions for injuries as a result of intercollegiate sports. Double check your policy to make sure you are covered. Insurance policies may be selected from various sources locally or online of your choosing. Pivot Health insurance policies may serve as an option and can be found at www.pivothealth.com or (877) 974-7462. Please note Texas Lutheran University has no official affiliation with Pivot Health and cannot guarantee their policy will not exclude athletic injuries.
